Of course you can but it strongly depends on the situation. Did you document what went wrong? Do you have photos? Did anyone else see the situation? Doctors pay big money for malpractice insurance because a lot of people will sue unecessarily. I'm not saying you don't have a case, just make sure you have enough documentation. Look online for malpractice lawyers and contact them to see if you have a case. Good luck.

He would have had to do something either not on your signed form or did something such as "screwed up" the surgery. The best thing to do, if you think he messed up, is take all your medical records to an attorney who should see you as a consultation for free. Many take cases based on a share of what you get in a hearing from a judge or jury. This does not occur quickly and it takes a lot to prove your case. Each state has different laws so some allow malpractice suites based on merit but limit personal injury which is usually things like inability to work etc. They usually have a way to appeal also.
