Dr. Gallagher explained the differences between medical errors and adverse events. Issues surrounding health policy, ethics, institutional responsibilities, inter-professional dynamics, team dynamics, education, training, counseling, and support for staff were also discussed. Mary K. Hardy, RN, MA, CCRN (Clinician III, Porter Medical Intensive Care Unit, University of Virginia Health System) addressed the roles, attitudes and experiences of nurses with medical error disclosures and highlighted the roles, policies and initiatives of institutions.
