What are the 4 D's of medical negligence and what can be done to avoid them?

Literally the 4 D's = Dereliction of a Duty Directly causing Damages. (Dereliction meaning "deliberate or conscious neglect.") The 4 D's are used when a case is presented against a physician/practice for negligeance/malpractice. The patient must provide proof against the physician for all 4 parts.
I'm am just starting to research this myself for a class discussion involving medical malpractice and negligeance.
