So, my sister is in a weird custody situation. Anyways, here's the deal. The hospital is claiming that my nephew hasn't been seen since July '05 and he was born Janurary '05. Now, I have been to countless appointments of his since then. We have copies of his immunization record and yet, the hospital is still claiming that he hasn't been seen for four years. He was last there in November.
His paternal grandmother is trying to nail my sister with child negligence for this reason. She currently has temporary custody and when she went to get his medical records the hospital (Kaiser, Oakland) said that they have no record of him except that one time in July.
What is going on here?
What can we do to prove that we have been taking him to the doctors and updating his shots? (aside from the immunization records.)
Also, they botched my nephews circumcision, and my conspiracy theory is that they're trying to cover up the fact that we've asked them to do something about it. Maybe I'm a bit off, but it's a little weird to me, that his brother's and sister's medical forms were intact and up to date, but his is the only one missing.

Mr. Warrior, I'm not going to say exactly why she doesn't have custody right now but it involves housing. She *is* a fit mother, and I know for a fact that she took them for shots, check ups, etc because I was there for a few of them. She always has either myself or her best friend with her.

You paid for immunisation? There could be a record that way. There has to be a file at the hospital also, they are usually not that slack. You just got to get a nurse to find it.
So why is the sister not having custody of her own child? She not a suitable mother. Think is it possible she did not take the child for immunisation.

First of all, with Kaiser you may be entering the hospital itself, but they do not do the billing for individual physicians. Best thing to do is go in where you took him, that very office and ask for a printout of visit dates.
If that doesn't work, odds are good your sister used some type of government assisted program which paid for the services. In California its called Medical if you are very low income and Healthy Children if you are working and income is too high for Medical. Her worker would be able to have the data that shows what payments were made to the provider on their behalf. It may take some digging and time to get the right person with welfare to assist, but certainly if they paid hospital, they would have record.
Now if for some reason she has private insurace, it's much easier. Call the company and request duplicate eob's(explanation of benefits). Normally these are sent out, but budget cutbacks have lead many companies to not send them out.
Last but not least, be sure and only give out copies of that immunization card. If you lose it, you have nothing. Its your documented proof someone at Kaiser saw him, even if it is only a shot clinic.
As for your conspiracy theory, more than likely someone in legal of hospital had his file pulled to read notes, not that it doesn't exist, just not where it's supposed to be so the answer of "we dont have record" may change when you go in person with that shot record.

You should contact an attorney about the botched circumcision. Contact arclaw.org. He's in California and I'm sure he can force the hospital to produce records.

The immunization reccords should be enough, plus the hospital should have reccords. Tell your sister to get a lawyer, they can help this a lot. Also tell your sister to get in contact with the doctors that he has seens in the past. If they testify they did a check up with him then she has enough proof. Plus if she gets that and proves the hospital has been failing to do reccords she can sue or press charges against the hospital.
I agree with you, they botched the job and want to cover it up. (most likely). The reason I have no trouble believing that is because I have heard this story a few times before with botched circumcisions.
