A lawyer would simply request the doctor's insurance info just like someone who gets into an automobile accident. The doctor may refer a lawyer to contact his attorney for info but it's a mere formality.

Actually, NONE. You would be suing the DOCTOR for malpractice. His insurance would pay any judgment and perhaps supply him an attorney. The limits on his insurance do NOT affect the damages you are entitled to sue for.
