Medical malpractice is very difficult to prove.
So first, you need to have a little knowledge of the law. If you're certain they are violating the law.
Call your hospitals administration, and get a copy of all of their procedures, in writing, so you have proof that there has been a violation made.
Then, you actually have to have solid proof, beyond reasonable doubt, that the people in question have committed these violations, of hospital policy and law. That's where it gets tricky. In most places you cannot be recorded or videotaped without your knowledge and consent. UNLESS it is a security measure, put in place by the hospital. So your best bet is to talk to a lawyer and have them request the CCTV tape, from the head of security at the hospital.Also, if there could be a paper trail of medical records not being kept properly, follow that. Also, be aware you need to do this quickly, because the tapes are usually not kept very long. Right now, those are your best steps to take.
If you run into a wall with finding the paper trail, or being able to prove what was actually violated, or who did it, call your hospital and tell them exactly what you suspect is going on. You can do it anonymously or you can give them your name. I recommend anonymously. Especially if you are going to give names, ect.

See a lawyer first. I also know in my heart to protect the safety of the patients I would want to call the ombudsman for the facility and the State Health Inspectors to check the place out to see if the place is running at adequate staffing, or abusing the patients at this time. They also look at many other things patients quality of life being a big part of it. (If this is a long term care facility we are talking about) If this is Hospital not really sure who to start with for inspections...start with Lawyer for sure.
