I'm not aware that physicians are "Required" to have malpractice insurance. That said, in the USA and the EU it would be stupid not to.
It the UK at least the only insurance that the law "Requires" is third party risks for my car. All other insurance is voluntary.

Ian M is right that malpractice insurance is not required by law, but it is for most practical purposes in the US if you want to work in a hospial or have any managed care patients (PPOs, HMOs) since the managed care companies and hospitals require it as a condition of working with them.
To my knowledge, no country in the world requires malpractice insurance by law.
