No they don't. It can vary widely depending on your experience, where you practice, and how much coverage you get.

Totally depends. They may be on the hospital staff. In other cases, anesthesiologists may be self-employed and carry their own malpractice insurance.

Depends on if they're an employee of the hospital or not. Employees, the employer pays for it (most likely the hospital).
The rate isn't straight - it varies wildly based on how many years they've been doing it (the price goes UP the longer you're doing it, due to the way those policies are written with a retroactive claims date), your claims experience, how many hours you work, and what type of anesthesia you do.

Just like car insurance is different for each person, malpractice insurance is different for each physician. We fill out an application that is about 15 pages long to get insured, and underwriters determine how much we pay.
It might be $ 20,000 a year, it might be 5 times that.
