bad dental care from dentist, unsanitary proceedures, dental work (fillings in a tooth has cracked in 1/2 and cavity that was suppose to be corrected has gotten larger, part of filling is intact the other half is floating up to the gum, and floats around loose.

I'm a California attorney (who doesn't practice in this area). Make sure to contact an attorney who specializes in medical/dental negligence, not just any attorney. There are many great specialists in this area--particularly in California.

If you want malpractice and for the dentist to get in trouble and loose his license (which is what I assume) try the ADA website, they will explain to you exactly how to file a malpractice claim. It would also be a good idea to contact a lawyer, there are lawyers that only take malpractice cases.
