I am confused on the difference between substantive and procedural law - I know that substantive is what gives rights/responsibilities and procedural is how a case is handled. But which would a medical malpractice lawsuit be?

Substantive of course. Your doctor has the responsibility to provide care at a certain standard. A lawsuit alleges it was not.
