a doctor noted that a patient was suffering from "iatrogenic" condition called hypernatremia. Patient did die. in the same doctors note it reports that the patient was liely suffering from vancomtcin nephrotoxicity, (which may have caused his kidney injury??) What is this all meaning? Did they give the patient something that caused the kidneys to fail? Did the doctors mess up?? Please tell me any information you can tell from this. Thank you.

Iatrogenic means "doctor induced." Apparently the patient died as a result of vancomycin (an antibiotic) nephrotoxicity (kidney failure due to the amount of vancomycin administered).

A medical malpractice case is based upon the following: Did the treatment that was provided fall below the standard of care which exists in the community where the physician practices?

Plus, in order to prevail in a med mal case one must have a medical doctor (preferrably one who practices in the same area of medicine as the defendant physician), who will testify that the treatment provided did in fact fall below that standard of care.

The doctor administered or ordered the medication but in doing so was he/she negligent in the standard of care? This is what must be proved in court.

Gather up the medical records and visit an attorney. If the attorney does not tell you exactly what I wrote above, politely leave and go see another attorney.
