The patient is a family member who needs dental treatment at a specially designed medical facility. Moreover, the patient would need to be taken via ambulance due to the severity of the patient's health condition. Further, the patient only receives medi-cal and medicaid due to low income.
To clarify, the nursing home staff are massively resisting because if the patient has just one (1) cavity, it would demonstrate negligence on their part. Therefore, they seek desperately to impede the dental treatment. Aside from Ombusperson or Public Health, what other agencies are available in receiving assistance in this matter to transport the patient in California??

Who (if anyone) is guardian over the patient?
If the patient is lucid, and can request transport and treatment, or if you are the guardian and request transport and treatment, then they are required by law. Contact your state agency, it won't take long to get it resolved.

What kind of medical facility is providing dental treatment for a nursing home patient? In California anyone over age 21 does not get any dental services under Medicaid. Medicare now has some dental benefit but very limited, definitely not an expensive facility you describe.
There is an advocate for people in nursing homes in California. Your problem is not transporting the patient. Your problem is getting the person's doctors permission to leave the facility and in paying for this dental treatment.

I think you need to find out if dental is even covered under Medi-cal or Medicaid. I don't think dental is covered which is why transportation to get any work done would also not be covered. Medicaid only pays for transportation to medical appointments they are paying for. I know it may not seem fair, but dental is hard to come by in this country unless you pay for a dental plan or pay for treatment out of pocket.
I agree with the others on here, dental problems would not demonstrate Negligence. People get cavity's and other dental problems even when they take really good care of their teeth. Also, someone who is in poor health to begin with is going to be taking medications which would further break down their teeth.
My advise to you is to find a dentist who is willing to come to the facility and you'll probably have to pay for it yourself. Sorry, if this isn't the answer you wanted to hear, but just being honest.
