This is the first YouTube segment of Primary Counselor Doug Dodge's deposition taken for the wrongful death/medical malpractice lawsuit, BURNE v THE MEADOWS. In 2004, and for numerous (20+) years prior, Dodge had been at The Meadows. In his capacity as a Primary Counselor, Doug Dodge was responsible for designing, coordinating, assessing, monitoring and supervising patient treatment plans at THE MEADOWS. He also did individual and group counseling. Educationally, Doug Dodge holds a GED (General Educational Development)diploma. No further academic credentials can be located or verified for him. Doug Dodge was grand-fathered into Arizona State Licensure as a Counselor in May of 2004. This permitted him to opt out of new AZ Department of Health State Licensing requirements that were put into effect on July 1, 2004. PLEASE NOTE FOR CLARIFICATION PURPOSES: At the 2:28 minute marker of this video, Doug Dodge states that Matt Burne was on "One-to-One" (observation). This comment is in error. At no time on the Friday, Saturday or Sunday preceeding his suicide was Matt ever placed on One-to-One or Eye-to-Eye observation. At the time Matt Burne walked out of the Acute Unit of this Level 1 Psychiatric Acute Hospital and completed suicide off premises, Matt Burne was being watched once every four hours (or Q4 observation). No staff knew that he had left or saw him leave the facility sometime between 3-6AM. Additional information for this video is currently pending. Please check back ...
