you need to put an affidavit before the MADA and you case of malpractice against the doctor and you have to give MADA all the Information about exactly what had taken place with the doctor and you you can not hide your name you just could be bias against this doctor or he or she could had an affair with him and things went sour between you and the doctor and you want to get back by reporting him if you don"t have a genuine case against him don't waste your time because you will not get any help MADA remember that never advance to something that you can not proof and not prepared to give your name.
